Hi, I’m Peter Abraham, BSN, RN. My life has not always been easy. Growing up with near homelessness, hunger, and instability taught me what it feels like to struggle without help. My personal journey and faith led me to nursing later in life, where I found my calling in hospice and palliative care.
Working with families facing serious illness showed me how hard it can be to understand care, coordinate doctors, and make decisions that honor your values. I knew there had to be a better way to support people before, during, and after life-changing health moments. That is why I founded Compassion Crossing—to provide guiding support throughout your health journey, including dementia care coaching and education.
My approach is straightforward: I meet people where they are, with no judgment and unconditional love. I help them navigate life’s significant changes, whether they’re healthy, living with chronic illness, facing terminal illness, or nearing the end of life.
My services range from advance care planning for individuals 18 and older to illness navigation for those with chronic or terminal conditions. I also offer grief support, including anticipatory, ambiguous loss, and post-loss care, as well as end-of-life care. Additionally, I provide medication reviews for safety and wellness checks for those living in my service area of Madison County, Kentucky (Note that most of my services can be delivered remotely).
